Fault Tree Analysis (FTA)

FTA is a tool for evaluating a device from a “top down” perspective. It consists of a logic diagram that starts with a hazard, and then identifies possible situations when hazard can occur. At it's most basic level, it consists of “and” and “or” gates. FTA is an excellent tool to find “single fault” conditions (a single failure that may result in hazard).
